Important Information About the Program:

THIS PLAN IS NOT INSURANCE.

The medical discount components of the Care Entrée program are NOT health insurance or a health insurance policy, but provide discounts at certain health care providers for medical services. The range of discounts for medical or ancillary services will vary depending on the type of provider and the medical or ancillary services received. The Care Entrée discount program does not make payments directly to the providers of medical services. The member is obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with the program. Care Entrée makes available, before purchase and upon request, a list of providers, including the provider’s name, city and specialty. Equal or lower prices may be available through individual negotiations. The Capella Group, Inc. is the operator of the Care Entrée medical discount program, with its corporate offices located at 4929 W. Royal Lane, Irving, TX 75063. Note: The program may not be available in all states and may vary in some states.  Prescription drug discounts are available only at participating pharmacies.

Refund Policy:

Members who cancel during the first 30 days of membership by notifying Care Entrée in writing and returning their ID cards will receive, within 30 days, a full refund of membership dues.  Any application/processing fee is not refundable (except in Arkansas and Tennessee).  Members may cancel after the first 30 days, and, in states where required, will receive a pro-rata reimbursement of all periodic charges. 

Association Membership:

The Care Entrée program is provided through a membership in America's Health Care Consumer Association (AHCCA), a national membership organization committed to helping consumers save money. As part of your enrollment in America's Health Care Consumer Association (AHCCA), you are automatically enrolled as a member of Care Entrée and of Consumer Assistance Services Association (CASA). Certain of your program features are available through membership in CASA. For some programs, active members are automatically included under the association's group insurance policy for certain defined insured features. The program is not intended to be a replacement or substitution for any health insurance coverage. AHCCA and CASA are not-for-profit national organization, committed to helping consumers save money. Sales representatives receive compensation from the association for promoting and selling memberships in the association. The association contracts with the providers of services. The association investigates these providers but cannot warrant or guarantee their performance. As always, the association invites and encourages your suggestions on ways its programs can be increasingly beneficial to its members. Membership in the association is not contingent upon health status. Each member, by applying for our program and maintaining membership in the program, hereby designates and appoints the secretary of the association in office at any particular time from this point forward as his or her proxy and agent and attorney-in-fact to receive all notices of meetings of the members, to attend and vote on his or her behalf at any and all meetings of the members, to execute consents and to otherwise act for the member in the same manner and with the same effect as if the member were personally present. Further, each member authorizes the proxy to substitute any other person to act under this proxy, to revoke any substitution, and to file this proxy and any substitution or revocation with the association. This proxy is a voluntary designated appointment and the member has a right to receive all notices of meetings of members and to attend such meetings and vote thereat. In such event, the member will notify the Secretary of the association of such desires.
